SERVICES RESIDENTS CAN EXPECT
  • A clean, secure residence for time determined at intake
  • Weekly meetings with Case Manager to discuss you individual goal achievements. (Person
    Centered Plan for Success)

  • Assistance in getting affordable permanent or transitional housing
  • Referrals to outside agencies for counseling and health care issues.
  • Life skill classes and training.
  • Opportunities for fun through organized recreational activities.
  • Opportunities to share ideas about program enhancement.

ADMISSION CRITERIA
  • Young homeless adults ages 18-23.
    Prior foster care youth and residents of High Point will be given priority.
  • Must be willing to submit to an initial drug test and future random testing.
  • Emancipated youths ages 16-17 will be considered.
  • Must sign a program contract and take the necessary steps to successful complete the agreed
    upon Person Centered Plan (exit plan)
  • Must have the capacity to work a full time job.
  • If High School Diploma / GED has not previously been attained, must be willing to enroll in an
    education / vocational institiution.
  • Must be willing to work towards achieving self-sufficiency and moving on to independent living within
    18 months.
  • Must be physically, mentally, emotionally and behaviorally ready to safely reside in the facility and
    use program services.
  • Willing to commit to staying in the program until all Person Centered Plan goals are met and able to
    be self-sufficient.
  • Must submit a criminal background history. Determinations of admission based on findings will be
    made on an individual basis.

Note: All selected applicants will be asked to take a drug test, provide criminal background history and
sign a PCP and contract upon admission.


Exclusionary Criteria
  • Active psychotic behavior
  • Violent / assaultive behavior
  • Active substance abuse rendering the young adults unsuitable to group process and interaction.
  • Verbal abuse

REFERRAL PROCESS
Referrals require active involvement by the young adult and case manger. The young adult must:
  • Request admission
  • Tour the facility and meet with staff
  • Meet with current residents
  • Read and understand the house rules and responsibilities

The case manager will be requested to complete the referral by giving Crossroad's staff the following if
available:
  • Children's Services Case Plan Evaluation
  • Ansell-Casey Life Skills Assessment
  • Social Summary
  • Medical clearance
  • Staff will make the final determination concerning admission. The first thirty (30) days of the stay will
    be considered a trial period. At the end of thirty days the young adult will be asked to evaluate the
    ability of the program to meet his needs and the staff will be asked to evaluate the fit of the person
    with I AM NOW, INC. (Crossroads).

YOUNG ADULT'S ASSESSMENT
The young adult's independent living competencies will be identified using the Ansell-Casey Life Skills
Assessments. The purpose of a life skills assessment is to provide the young adults with preparing for
self-sufficiency. The assessment is used to be in conjunction with other information available to focus
specifically on those areas of the young adult's life that are, or will, impact on their preparation for self-
sufficiency. The assessment will assist in gaining understanding of the young adult's strengths and
challenges so practical, concrete efforts can be made to achieve the goals to which the young adults
seeks.

I AM NOW, Inc. is built on the "Person Centered Concept", which identifies the young man as the key central figure
in his "Exit Plan".  The "Exit Plan" is a myriad of skills, services, and support networks that are combined and
overlapped to sustain the basic needs of the resident. Mentor/Mentee relationships will also be developed to offer
support, behavior modeling and assistance with the identification, development and/or linking together of support
services specific to each individual resident's needs.

The goal is to assist the resident from the point he is NOW to develop the "Exit Strategy" to leave the transitional
living program and move into his future as a productive member of the community.

We Serve Homeless Young Adults aged (18-23) who:
  • may have broken family ties
  • may have a history of drug use
  • may have a history of criminal behavior
  • may have dropped out of high school
  • may be victims of abuse
  • may have aged out of foster care

We accept referrals from schools, churches, direct service providers, adult emergency shelters, and our local
Department of Social Services.
